Below is a checklist of documents that we require to enable you to register with us:
New Candidate Registration Checklist
- Application pack documents which include:
- Application Form
- Candidate Consent Form (GDPR)
- Candidate Health Questionnaire
- Candidate Consent and Authorisation Form
- HMRevenue Check list doc.
- Working Time Regulations
- Passport size Photograph
- A copy of Immunisations (if applicable)
- A copy of Nursing Midwifery Letter with Pin no (nurses)
- Details of Professional Body (if applicable)
- Certificate of Incorporation or UTRL Number – (if Limited Company or self-employed)
- Proof of right to work (passport/residence permit)
- A copy of current DBS
- Copies of current mandatory training certificates
- Two proofs of current address (must be less than three months old)
- A copy of your national insurance number
- A copy of your Driving License and Counterpart (if applicable)
