New Candidate Registration Checklist

  • Application pack documents which include:
  1. Application Form
  2. Candidate Consent Form (GDPR)
  3. Candidate Health Questionnaire
  4. Candidate Consent and Authorisation Form
  5. HMRevenue Check list doc.
  1. Working Time Regulations
  • Passport size Photograph
  • A copy of Immunisations (if applicable)
  • A copy of Nursing Midwifery Letter with Pin no (nurses)
  • Details of Professional Body (if applicable)
  • Certificate of Incorporation or UTRL Number – (if Limited Company or self-employed)
  • Proof of right to work (passport/residence permit)
  • A copy of current DBS
  • Copies of current mandatory training certificates
  • Two proofs of current address (must be less than three months old)
  • A copy of your national insurance number
  • A copy of your Driving License and Counterpart (if applicable)
